Job Description

Position Summary • Under the supervision of the Hunterdon New Jersey Cancer Education Project Coordinator, the Community Health Worker will provide support for cancer screening grant initiatives, including but not limited to outreach goals, client enrollment, health data entry, data reporting, and basic navigation to age-eligible uninsured, underinsured individuals, identifies and helps to reduce barriers to care, provides referrals to necessary resources, for breast, cervical, colorectal and prostate cancer screenings services Primary Position Responsibilities 1. Community outreach 2. Case management & patient navigation 3. Identifies and reduces client barriers to care 4. Community advocate 5. Monitors client outcomes 6. Health data entry 7. Monthly reporting
Qualifications Minimum Education:
Required:
• High school diploma or equivalent
Preferred:
• College degree in health/social service related field Minimum Years of Experience (Amount, Type and Variation):
Required:
• Minimum of one year of community involvement and/or working with clients
Preferred:
• Equivalent combination of health/social service education and experience
License, Registry or Certification :
Required:
• None Knowledge, Skills and/or
Abilities :
Required:
• Candidate must reside in New Jersey. Knowledge of community resources and medical terminology, strong written and verbal communication skills, including public speaking to small and large groups; proficient computer skills. Ability to work independently and as part of a team. Strong organizational and interpersonal skills. Valid NJ driver's license and reliable transportation
Preferred:
• Familiarity with Hunterdon County and surrounding communities. Experience with case management. Bilingual (Spanish/English) preferred. Comfort driving around Hunterdon and surrounding counties. Experience with Excel spread sheets, Microsoft Word, and other data management software programs, Google and Electronic Medical Record (EMR). • This position description is not intended to be all-inclusive, and the employee will also perform other reasonably related business duties as assigned by the immediate supervisor and other management as required.
