Duties Conduct outreach to community members to assess their health needs and connect them with appropriate services Provide education on topics such as nutrition, medication management, addiction counseling, and preventive health measures Assist individuals with disabilities or chronic conditions in navigating healthcare systems and accessing resources like Medicare Use motivational interviewing techniques to encourage positive behavioral changes and improve health outcomes Collect and document data related to community health trends, service utilization, and client progress Support public speaking engagements to raise awareness about health programs and available services Collaborate with healthcare providers, social workers, and community organizations to coordinate care plans Qualifications Prior experience or training in social work, public health, or related fields is highly desirable Strong understanding of medical terminology and healthcare systems Experience working with individuals with disabilities or those facing social barriers Excellent communication skills for teaching, public speaking, and motivational interviewing Ability to collect accurate data and maintain detailed records of interactions and services provided Knowledge of nutrition principles and addiction counseling techniques is a plus Demonstrated commitment to community service with a proactive attitude towards health promotion Bilingual in Spanish and or Portuguese preferred This role is designed for motivated individuals eager to foster healthier communities through compassionate support, education, and advocacy.
