How you will make an impact : The Community Health Worker plays a trusted, frontline role connecting clinical care teams with the people and neighborhoods we serve. Drawing on lived experience and genuine community knowledge, the CHW addresses social determinants of health, supports care coordination, and helps patients access the resources that lead to better health outcomes.
What you will do:
Patient Outreach and Engagement Reach out to underserved individuals and families to build relationships and connect them to care. Conduct community site visits and phone follow-ups to support patient engagement and progress on care plans. Help patients navigate appointments, transportation, insurance enrollment, and other barriers to accessing services. Maintain a patient caseload with regular follow-up and thorough documentation of all contacts. Social Determinants of Health (SDOH) Administer SDOH screenings using validated tools such as PRAPARE or
AHC-HRSN.
Identify unmet social needs, including housing instability, food insecurity, employment, childcare, and utility assistance. Connect patients to community resources and facilitates warm handoffs to social service partners. Care Coordination and Team Integration Work alongside clinical and administrative staff, contributing to team huddles and care conferences. Communicate patient needs and updates accurately and promptly. Support chronic disease management, preventive health programs, and population health goals. Facilitates referrals and follows up to confirm completion and patient satisfaction. Health Education and Community Engagement Deliver health education that is culturally and linguistically appropriate for the populations served. Lead or support community health events, workshops, and outreach activities. Serve as a community ambassador for the health center, building trust and visibility among the communities we serve. Documentation Maintain accurate, timely records in the Electronic Health Record (e-Clinical Works) and other tracking systems. Meet program productivity and reporting expectations. Participate in ongoing training, quality improvement, and compliance activities. Who you are and what you will bring: Education and Experience High school diploma or GED; an Associate or Bachelor's degree in public health, human services, or a related field is a plus. CHW certification preferred; candidates without certification who are willing to pursue it within 12 months of hire are welcome. At least one year of experience in community health, social services, or patient navigation is preferred. Lived experience within the community served is strongly valued and considered an asset. Knowledge, Skills, and Abilities Bilingual proficiency in Spanish/English is a plus. Strong interpersonal, communication, and active listening skills. Familiarity with local community resources and social service systems. Comfort with Microsoft Office. Valid driver's license and reliable personal transportation for field-based work. Pay:
$21.00 - $24.00 per hour
