$15.00-$16.00 per hour DOE Nonviolence Works, Inc./Taos Behavioral Health is actively seeking compassionate and dedicated individuals with a background in human services to join our team as a Certified Community Health Workers (CCHW) - Family Navigator connects families to essential healthcare and social services. You will serve as a trusted liaison between the community and medical, behavioral, and social service systems. Your primary goal is to help families overcome barriers to care, improve health outcomes and build self-sufficiency. At Nonviolence Works, Inc., we understand that providing excellent care requires the right resources, tools, and support for our staff. We are committed to ensuring you have everything necessary to serve our clients effectively and compassionately. As our agency expands to meet the evolving needs of the community, including school-based services, this is an exciting time to join our outpatient office.
Job Description and Duties:
    Family Advocacy:
    Conduct outreach to identify families needing support services.
      Needs Assessment:
      Evaluate family strengths, challenges, social determinants, and resource gaps.
        Care Coordination:
        Guide families through complex healthcare and social service systems.
          Resource Connection:
          Link families to housing, food, insurance, and childcare.
            Goal Planning:
            Develop individualized, family-centered goal plans with measurable steps.
              Health Education:
              Provide culturally appropriate education on wellness and chronic diseases.
                Appointment Support:
                Help families schedule, attend, and prepare for medical appointments.
                  Home Visitation:
                  Conduct home visits and community meetings to assess environment needs.
                    Data Tracking:
                    Document all client interactions, assessments, and referrals in the EHR.
                      Community Collaboration:
                      Build relationships with local non-profits, clinics, and school districts.
                      Required Qualifications:
                      Active state Certified Community Health Worker (CCHW) High School diploma or equivalent general education degree Successful completion of an approved Community Health Worker Certification Training Program. 18 years or older Able to pass a Federal Background Check Able to pass a Drug Test/Drug Free Valid Driver's License / Insured personal vehicle.
                      Preferred Qualifications:
                      Experience using Electronic Health Records (EHR) or Social Service Databases. Must be able to sit for stand for prolonged periods. Must be able to walk, climb stairs, and carry up to 20 pounds during home visits. Fluency in English and a secondary community language (e.g; Spanish, Arabic)
                      Experience:
                      • Minimum two year working directly with underserved families or communities.
