Job Description

Position Summary The Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. CHW is responsible for identifying patients served by NVHC and monitoring selected patients to ensure compliance with screenings and treatment plans. The CHW will participate in training programs to learn the best-practice methods for effective patient navigation and/or education. Provides training for clinical staff regarding special grants and projects for patients. Nevada Health Centers is an employer of choice and offers comprehensive benefits, including but not limited to: Competitive compensation, with annual merit increases Company sponsored PPO medical plan and buy-up option, vision and dental plans 403(b) retirement plan with employer match Generous time-off policy, and ten (10) paid holidays plus one (1) float holiday Company paid life/AD&D and long term disability policies Essential Duties and Responsibilities Promote health by educating patients regarding prevention and/or treatment of healthcare needs Coordinate healthcare screenings for assigned measures per the needs of the patient Track patient follow up care and patient no shows, promoting continued care Provide financial and medical community resources for patients Provide pre-visit planning to ensure patients understand what to expect during their appointment and account for any outstanding patient results Contact assigned insurance patients to inform them of services at NVHC and schedule appointments with providers Works closely with providers to receive guidance and instruction for patient care follow-up Utilize electronic medical records system for reporting, patient data entry, documenting services provided Provide training and education to clinical staff regarding patient resources and focused quality measures Follows all policies and procedures and appropriately report concerns and patient issues as well as procedures for personal requests. Alert for ways to improve customer service, improve patient flow Attend community events to promote care opportunities available at NVHC Required Qualifications High school diploma or general education degree (GED) CHW Certificate of Completion or ability to obtain certification within 6 months from date of hire. BLS/CPR Certification Must be able to attend community events, conferences, and off-site meetings Preferred Qualifications English/Spanish fluency Demonstrated proficiency with computer programs and data entry, experience with Electronic Health Records highly preferred Experience in a healthcare setting with diverse populations Knowledge of CPT and
ICD-9/10-CM
coding and HIPAA Ability to communicate via written and verbal communication Ability to exercise good judgement in appraising situations and responding appropriately to underserved population groups.
