Job Description

The Fallon Tribal Health Center is seeking a compassionate and community-focused Community Health Representative (CHR) to serve as a liaison between the health center and the community. This position provides culturally appropriate health education, patient navigation, care coordination, outreach services, and non-emergency transportation to improve access to healthcare and overall health outcomes. The CHR supports chronic disease prevention and management while helping patients access medical and community resources and overcome barriers to care. Essential Duties and Responsibilities Provide culturally appropriate health education to individuals, families, and groups on topics including chronic disease prevention and management, nutrition, physical activity, medication adherence, and wellness. Conduct community outreach activities, home visits, wellness programs, and community engagement events. Assist patients with appointment scheduling, referrals, discharge planning, and follow-up care. Help clients identify and address barriers to healthcare, including transportation, housing, and food access. Support patients in understanding diagnoses, treatment plans, medications, and provider instructions. Participate in health promotion, screenings, disease prevention initiatives, and community wellness programs. Document client interactions, referrals, services, and outcomes in electronic health records and program reporting systems. Collect and report data to support program goals, grant requirements, and quality improvement initiatives. Collaborate with healthcare providers, behavioral health staff, nurses, social services, and multidisciplinary care teams. Serve as a patient advocate and liaison between clients, families, healthcare providers, and community resources. Perform basic non-diagnostic health screenings under appropriate supervision and training. Provide safe and professional transportation services for health center clients and accompany patients to appointments as needed. Maintain accurate travel and transportation records and report vehicle maintenance concerns. Deliver medications to patients as needed. Maintain strict confidentiality of patient information and comply with all applicable privacy regulations. Perform additional duties as assigned. Qualifications High School Diploma or GED required. Must possess or obtain Nevada Community Health Worker (CHW) certification within 6 months of hire. Current CPR and First Aid certification preferred or willingness to obtain certification required. Must complete required driver safety and non-emergency medical transport training. Experience in community outreach, healthcare, case management, or a related field preferred. Valid Nevada Driver's License (or ability to obtain within 90 days of hire) and ability to remain insurable under the Tribe's vehicle insurance policy required. Knowledge, Skills, and Abilities Strong communication and interpersonal skills. Ability to build trust and maintain effective relationships with patients, families, and community members. Knowledge of community health, chronic disease prevention, care coordination, and advocacy principles. Ability to provide culturally appropriate and trauma-informed services. Proficiency with electronic health records, documentation, and data entry. Understanding of social determinants of health and available community resources. Ability to maintain confidentiality and comply with billing, compliance, and program requirements.
Compensation Pay Range:
$18.27 - $27.63 per hour
