The core focus of each CHW is to support the clinical care team in achieving the health outcomes of patients and to advocate on behalf of the patient and their family. These health outcomes can range from social determinants to clinical utilization, education/prevention. The CHW will help each patient/member and their families navigate healthcare, community resources and benefits. The Renown CHW will be integrated within the care team with responsibilities to appropriately document within the Electronic Medical Record (EMR), effectively communicate in a HIPAA compliant manner, and keep the strategic goals of Renown Health at the core of their work. Nature and Scope The Community Health Workers will be highly skilled in the following areas: a. Communication b. Interpersonal Relationships c. Knowledge base about the community, health issues, and available resources d. Service Coordination e. Capacity Building f. Advocacy g. Teaching and Education h. Organization i. Creating a high level of service and relationship-based experience for patient. k. Organizing and maintaining priorities and schedules to ensure deadlines are met. The Community Health Worker will educate clients and their families on the importance of lifestyle change, including adherence to their medication regimes, recommended treatments and how and when to access the right appropriate level of care. The Community Health Worker will help patients navigate the healthcare system, which includes making post-acute follow-up appointment, assisting them with enrollment eligibility, appointments, referrals, and transportation to their health care appointments. It also includes promoting continuity of care and obtaining the right level of care when needed. The Community Health Worker will be responsible for non-clinical case management. This includes community and health plan referrals, documentation of activities in the EMR, report management, accurate and detailed note taking, and persistent follow up. While managing patients, the Community Health Worker will need to conduct non-clinical assessments pertaining to their health determinants: social, environmental, lifestyle and appropriate level of care. Based on the care plan for each patient and the social determinant gaps needing to be addressed the CHW will connect patients to the appropriate community, social, and medical resources needed to address each gap and need. In managing each patient, CHW's will need to meet the patient in-person which may include community visits to help facilitate trust and effective engagement. Maintains a complete and smooth flow of information and communication between the patients, pharmacies, clinical teams and internal and external stakeholders Assists with special projects/duties as requested. There are clinical and community environments that the CHW may work in which could include, but are not limited to: unpleasant odors, uncleanliness, pets, infestations, or uncomfortable climate (i.e., warm or cool temperatures)

Preferred Name Description Education:
Must have working-level knowledge of the English language, including reading, writing, and speaking English. AA or AS degree in related field preferred. Preference will be given to individuals with degree in health-related field. Bilingual (Spanish) is preferred.
Experience:
1-2 years' experience in hospital or outpatient services or practice management; or minimum of 1-year direct experience in health care contact/call center, hospital or outpatient environment preferred. Population Health experience preferred. License(s): Valid State of Nevada or California driver's license and ability to pass Renown Health's Department of Motor Vehicle Report criteria. (Not required in cost center 530704). Required for this position Fingerprints must be able to pass Nevada Division of Public and Behavioral Health (DBPH) background checks upon hire and every 5 years per State of Nevada Revised Statue (NRS 449.123) to remain in this position.
