Job Description

  • Social Services Navigator / Community Health Worker — Wurtsboro, NY
  • In-Office + Community Outreach | Full-Time | $18-$23/hr | Immediate Start

  • We are opening a new community health and social services office in Wurtsboro, NY and we are looking for 2 compassionate, motivated individuals to join our team from day one. You will go door to door, sit with families, check blood pressures, help people get the benefits they deserve, and connect Sullivan County residents to healthcare and social services that change lives.
  • What You'll Do:
  • Help residents apply for Medicaid, SNAP, HRA, SSI/SSDI, and government benefits
  • Conduct community outreach — door to door, home visits, community events
  • Perform basic health screenings — blood pressure, weight, diabetes checks
  • Connect residents to healthcare providers, mental health services, and specialists
  • Assist with chronic disease management support — diabetes, hypertension, asthma
  • Follow up with clients after hospital discharge to prevent readmissions
  • Conduct intake interviews, assess social needs, and maintain case records
  • Make warm referrals to housing, legal aid, food assistance, and social services
  • Be a trusted, caring presence for vulnerable community members
  • You Are Perfect For This Role If You:
  • Genuinely care about people and want to serve your community
  • Are organized, reliable, and compassionate
  • Are comfortable going door to door and meeting people where they are
  • Have experience in social services, community health, case management, home health, outreach, or human services (preferred but not required — we will train the right person)
  • Hold or are working toward CHW certification (a plus but not required)
  • Are bilingual English/Spanish (a strong plus)
  • Live in or near Sullivan County — Wurtsboro, Monticello, Middletown, Liberty area
