- This is a full-time field-based position in Ohio.
Fundamental Components:
Conduct home visits with Aetna members with behavioral health conditions to assist them with their care plan including identifying the options that best meet the needs of the member, adjusting tasks to fit with the member's culture, language, and religion, improving access to care, identifying behavioral health care needs, and reducing unnecessary PRTF hospitalizations to help the individual obtain positive behavioral health goals. Educate healthcare professionals and providers on opportunities for improving and understanding the social determinants of health that may be impacting member's health and treatment plan through various communication channels. Build and maintain positive working relationships with the Aetna members, providers, behavioral health case managers, agency representatives, supervisors, and office staff, from diverse cultural and socio-economic backgrounds. Work to reduce cultural and socio-economic barriers between clients and institutions. Assist members to identify socio-economic issues that affect their overall health and develop health/social management plans and goals. Identify gaps and opportunities to strengthen systems of care within the member's community and assists members in utilizing community services, including scheduling appointments with social services agencies, (including transportation vendors), and assisting with completion of applications for eligible programs. Assist with patient medication adherence by educating and instructing the member on current medication list, reviewing medications with member and assist in obtaining refills, and identifying alternatives if necessary. Required Qualifications Complete an approved CHW certification program within 12 months of hire. Classes count as work time and is provided/reimbursed by Aetna/CVS/ Demonstrated proficiency with personal computers, keyboard, multiple systems navigation, and MS Office Suite applications (Outlook, Word, Excel, SharePoint). Experience with children/adolescent populations. Ability to travel up to 75% of the time to multiple office locations, home locations within the region and occasional travel outside of the region. Travel is with personal vehicle. Must possess reliable transportation, valid/active driver's license, and proof of vehicle insurance. Requires flexibility to work in situations in which the working conditions are unknown, as environments and situation of patient homes may vary. Ability to communicate medical and mental health information to health care professionals and care coordinators over the telephone. Knowledge of formal and informal community agencies and resources. Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services. Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Able to influence mental health care providers as needed to adjust interventions based on member's cultural, ethnic, or religious preferences. Ability to work collaboratively in a team and manage multiple priorities, utilizes effective time management skills, and exercise sound judgment. Exceptional organizational and interpersonal skills, with attention to detail required. Ability to effectively provide support care to socially and behavioral health complex patients in a variety of non-traditional settings. Preferred Qualifications 3-5 years' relevant work experience specifically working in a community-based position or in a mental health capacity. Experience completing CANS Assessments (Child and Adolescent Needs and Strengths) Experience working with youth involved in the Juvenile Justice System.Preferred skills:
Mental Health First Aid, Trauma Informed Care, HIPPA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions. Experience in serving the Medicaid Populations in urban or rural environments with familiarity of local formal and informal resource networks preferred. 1-2 years' experience providing clinical services; experience in community/outpatient setting preferred. Community Health Worker Certification or equivalent training and experience preferred.- CHW certification can be obtained within 12 months of hire. Experience with Electronic Health Records preferred. Meet all requirements set forth by local and state (i.e., first aid, CPR, TB screen, etc.) Bachelor's Degree Preferred Education High School Diploma or G.E.D.
