Job Description
This position is located in Nowata, Oklahoma Implements effective strategies for linking uninsured and underinsured clients to primary care, screens for social determinants of health needs (including food, housing, transportation, utilities, and safety), increases access to appropriate medical care and community-based services, assists individuals in improving health status, and promotes behavior change in using Morton health care services.
Requirements:
High School Diploma or GED required. Healthcare, social work, or public health background preferred but not required. Understanding of the healthcare delivery process is required. Excellent interpersonal communication skills are required. Broad knowledge of community resources and services is required. Excellent computer skills are required Required to maintain a high level of confidentiality. Demonstrated ability to work with community partners, patients, and staff professionally and cooperatively. Ability work in a fast-paced environment. This position has been deemed a" Safety-Sensitive" position; therefore, Morton reserves the right to refuse to hire an applicant or may terminate an employee in a "Safety-Sensitive Position" who tests positive for medical marijuana even if they have a valid medical marijuana license.
