Schedule:

Monday-Friday, 8:30 AM-4:30 PM The Community Health Worker Liaison supports individuals and families by connecting them to healthcare, human services, and community resources. The Community Health Worker Liaison serves as a bridge between community members and service providers, helping reduce barriers to care by addressing Social Determinants of Health (SDoH). This role focuses on outreach, education, case management, and collaboration to promote independent, healthy, and recovery-focused living. Community Health Worker Liaison Key Responsibilities Serve as a liaison between community members and healthcare and human service providers Conduct outreach and assessments to identify health and SDoH needs Assist individuals with navigating healthcare systems and enrolling in services Provide case management, follow-up, and referrals to ensure needs are met Maintain accurate documentation and records Complete SDoH screenings and develop individualized support plans Conduct home or apartment inspections, including light cleaning and stocking when needed Educate community partners and stakeholders through meetings, presentations, and outreach Attend required trainings, meetings, and travel as needed Promote equity, inclusion, and cultural humility Perform other duties as assigned Community Health Worker Liaison Qualifications Experience in community health, human services, or a related field Strong communication, interpersonal, and organizational skills Ability to manage multiple priorities and work independently or as part of a team Positive, empathetic, and solution-focused approach Community Health Worker Liaison Education & Experience Bachelor's degree in a related field preferred Community Health Worker Liaison Computer Skills Proficiency with Microsoft Word, Outlook, Excel, and general data entry Community Health Worker Liaison Conditions of Employment Indiana County residency or strong community ties preferred Ability to work evenings and weekends as needed Valid driver's license and reliable vehicle (mileage reimbursed at federal rate) Ability to obtain required background clearances Willingness to obtain Community Health Worker certification (training provided) Additional Details
