Job Description

The Women's Help Center is seeking a full-time Community Health Worker (CHW) based in Johnstown, Cambria County. Position Summary The Community Health Care Worker (CHW) will work directly with individuals receiving services, family members, providers and other stakeholders, at the direction of a supervisor. Candidates must have a strong understanding of the Cambria County community. CHWs serve as an essential link/between health services, social services, and the community as they facilitate access to services, improve health outcomes, and enhance the quality and cultural competence of service delivery. CHWs build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, social supports, and advocacy. The CHW will provide care coordination services to address housing and other needs of individuals enrolled in the program. This position will include regular telephonic and face-to-face in-home contact with individuals receiving services, family members and other stakeholders to engage in problem-solving and referral strategies; community-based services; resources for housing and resources available to members to promote recovery and independence. The CHW also represents the organization to other professionals and the general public. This CHW position will focus on working with individuals who are housing insecure for various reasons in subsidized housing, with a focus in Johnstown. The ideal candidate will have expertise in issues such as housing, transportation, peer support, informal counseling, career training, and social supports.
Responsibilities:
Flexibility in schedule to meet the individual's need Engages in setting goals with individuals and provides assistance and connections with services, tools, and resources Assists in scheduling appointments as needed Connects the individual to a variety of community-based services and supports In-home visits with individuals and travel throughout the region with mileage reimbursement Advocates for the individual's medical, behavioral health, and social support needs Empowers individuals to self-advocate and follow through with treatment (making and attending scheduled appointments, setting goals, and community networking) Utilizes motivational interviewing to effectively identify the individual's strengths, needs, motivations, and goals in managing life circumstances. Works in the community setting at least 80% of the time, to have direct contact with individuals, support teams, treatment providers, and recovery supports Assists individuals with specific housing and non-medical needs that affect health and access to care Requirements High school diploma or GED 1 year providing customer service, human services, case management experience in the community required Strong preference for a background in social services and/or mental/behavioral health. Driver's license and reliable means of transportation is necessary for this role. A person in the community who has expertise in issues such as housing, transportation, peer support, career training and community education. About the
