Job Description

Job Summary We are seeking a dedicated and compassionate Certified Community Health Worker to join our dynamic team. In this vital role, you will serve as a bridge between healthcare providers and the community, empowering individuals to achieve better health outcomes. Your expertise will help address social determinants of health, promote wellness, and support vulnerable populations through personalized education, advocacy, and resource navigation. Responsibilities Engage with community members to assess their health needs and develop personalized care plans Provide education on topics such as nutrition, medication management, addiction counseling, and preventive health strategies Assist individuals with disabilities by coordinating services and facilitating access to healthcare resources Conduct motivational interviewing to encourage positive behavioral changes and enhance patient engagement Collect and document data related to health outcomes, service utilization, and social determinants of health for program evaluation Educate clients about Medicare benefits, medical terminology, and available community resources Advocate for clients within healthcare systems and connect them with appropriate social services and support networks Requirements Certification as a Community Health Worker or equivalent credential from an accredited program Experience working with individuals with disabilities or in social work environments Knowledge of nursing practices, public health principles, and medical terminology Strong communication skills with the ability to speak publicly and deliver educational presentations Familiarity with data collection methods and maintaining accurate records Ability to apply motivational interviewing techniques effectively Background in teaching or health education is a plus; experience in addiction counseling or nutrition is desirable Join us in fostering healthier communities by providing compassionate support, education, and advocacy.
