About the Community Health Worker Role As a Community Health Worker within our Geisinger team, you will utilizes a holistic approach, to encourage access to healthcare to meet medical/physical/ psychological health needs and promote the general well-being of referred Geisinger Health Choices Members. The emphasis is to increase awareness of preventative health care measures, educate on the proper use of emergency departments, and the benefits of establishing a relationship with primary doctors. Services are provided in accordance with all existing policies, procedures, and regulations and in accord with the mission and values of the company.
Starting Rate:
$20.00/hour
Community Health Worker Core Responsibilities:
Provide health education and outreach to community members. Assist individuals in accessing healthcare services and resources. Support patients in managing chronic conditions and navigating healthcare systems. Collaborate with healthcare providers and community organizations. Promote wellness and healthy lifestyles through education and advocacy. What You Bring Have at least three (3) or more years of experience in a social service or a healthcare-related setting have a bachelor's degree in Social Work, Psychology, or other related fields with practicum experience Community Health Worker certification Ability to read and write in Spanish Ability to work independently and manage a dynamic caseload Strong interpersonal, organizational, and communication skills A valid driver's license and reliable transportation Who Thrives in This Role We've found that people who are: Empathetic and adaptable Collaborative and communicative Self-motivated and reliable Team players with a deep sense of accountability …tend to thrive in this role.
