Job Description

Overview The Community Health Worker (CHW) plays a vital role in bridging the gap between healthcare providers and the community. This position involves working closely with individuals, families, and groups to promote health education, access to services, and overall well-being. The CHW will engage with diverse populations, particularly focusing on those with disabilities and other vulnerable groups, to provide support, resources, and advocacy. Duties Facilitate communication between healthcare providers and community members to ensure understanding of health services. Complete intakes, comprehensive assessments, Family Needs Assessments, and follow up as applicable. Conduct outreach activities to educate the community about available health resources and programs. Assist individuals in navigating healthcare systems, including Medicare and public health services. Provide support in areas such as nutrition, addiction counseling, and general wellness. Collect data on community health needs and report findings to inform program development. Utilize motivational interviewing techniques to encourage behavior change and empower individuals in their health journey. Teach health-related topics through public speaking engagements or workshops tailored to community needs. Collaborate with social workers and other professionals to develop comprehensive care plans for clients. Skills Proficiency in nursing practices or experience in a healthcare setting is beneficial. Experience working with people with disabilities to ensure inclusive support. Background in social work or public health is advantageous for understanding community dynamics. Strong teaching skills for effective health education delivery. Familiarity with motivational interviewing techniques to foster positive client interactions. Excellent public speaking abilities for engaging diverse audiences. Competence in data collection methods for assessing community health trends. Knowledge of medical terminology relevant to patient care and communication. Understanding of Medicaid processes and regulations to assist clients effectively. Awareness of nutrition principles that contribute to overall health improvement. Skills in addiction counseling to support individuals facing substance use challenges. This role is essential for enhancing community health outcomes through direct engagement, education, and advocacy.
