(LINC) is seeking a compassionate and community-focused Community Health Worker (CHW) to provide outreach, engagement, and supportive services to justice-involved individuals participating in the Pathways to Community Program funded by the Governor's Crime Commission (GCC). This role serves as a bridge between participants and critical community resources, helping individuals navigate behavioral health, substance use recovery, housing, employment, and reentry services. This position is part of a grant-funded program. Employment is expected to continue through September 2027; however, continued employment is contingent upon grant funding and is not guaranteed beyond the life of the grant.

Key Responsibilities:

Conduct proactive outreach and engagement with high-risk individuals in the community Build trusting relationships and identify early warning signs of crisis or instability Provide trauma-informed crisis intervention and de-escalation support Connect participants to behavioral health, substance use treatment, housing, and employment resources Assist with safety planning, intake, screenings, and individualized service plans Support participants in navigating systems of care and reducing barriers to services Maintain regular participant contact to encourage engagement and retention Document services accurately and collaborate with Case Managers and community partners Support group activities and Cognitive Behavioral Interventions (CBI), as assigned

Qualifications:

High school diploma or equivalent required Associate's or Bachelor's degree in a human services field preferred Community Health Worker and/or Peer Support Specialist certification preferred or willingness to obtain certification Experience working with justice-involved individuals, underserved populations, or individuals with behavioral health/substance use challenges preferred Strong ability to connect individuals to community resources and navigate service systems

Pay:
