Job Description
Job Overview The Benzie-Leelanau District Health Department is looking for a Community Health Worker (CHW) to split their time between two programs: A Benzie County school-based program and the health department's Community Connections program. The school-based CHW will join our School Wellness Program teams to promote health among school children through: Classroom/group presentations on health-related topics Development and distribution of health-related messaging Assisting students with chronic health conditions to access needed health care Assisting families in navigating community supports/resources through home/in-person visits, virtual visits, and telephone calls. As part of the Community Connections team, the CHW assists adult community members in Benzie, Leelanau, and Grand Traverse County by providing information and assistance navigating community resources through home/In-person visits, virtual visits, and telephone calls.
Qualifications:
Required:
High school diploma or equivalent Strong communication skills: Ability to communicate effectively with individuals of all ages and backgrounds, and the ability to present to groups from elementary age to adults. Proficient at using Microsoft Office applications, and comfortable learning and using unfamiliar electronic platforms. Proficient at using creativity software to create printed materials that are age-appropriate and engaging. Ability to build trust with individuals from diverse backgrounds. Strong organizational skills, and ability to manage a variety of tasks and complete work by established deadlines. Comfortable communicating with professional partners, including health care providers, community resource agencies, and school staff to advocate for and coordinate client needs and services. Community Health Worker certification, or willingness to obtain certification (we will cover all costs and provide work time to complete certification class) Current drivers license with reliable, insured vehiclePreferred:
