Responsibilities Engage with community members to assess their health needs and provide culturally sensitive support Educate individuals on health topics such as nutrition, addiction counseling, Medicare, and preventive care strategies Conduct outreach activities to promote health programs and services within the community Assist clients with navigating healthcare systems, including scheduling appointments and understanding medical terminology Utilize motivational interviewing techniques to encourage positive lifestyle changes and adherence to treatment plans Collect and document data related to client interactions, health outcomes, and service delivery for program evaluation Collaborate with healthcare providers, social workers, and community organizations to coordinate care and resources Qualifications Experience or background in nursing, social work, public health, or related fields is highly desirable Demonstrated ability to work effectively with individuals with disabilities or complex health needs Strong communication skills for public speaking, teaching, and motivational interviewing Knowledge of medical terminology, Medicare programs, nutrition principles, and addiction counseling approaches Ability to collect accurate data and maintain detailed records in compliance with privacy standards Passion for community service with a proactive attitude towards health education and outreach Valid driver's license preferred for community visits; flexibility to work evenings or weekends as needed Must have and Ohio Community Health Worker Certification Join us in creating healthier communities through compassionate engagement and expert support.
