The program provides interventions to impact the social and environmental barriers to health for clients in multigenerational households, to include children and older adults, and at least one of these indicators: frequent utilizers of emergency health services, those with frequent hospital admissions, and/or not meeting managed care standards for their medical condition(s) (focus on Hypertension, Diabetes, HIV, and Asthma). The Community Health Worker is responsible for conducting the program's service activities and monthly internal case consultations, regularly submitting service documentation and monthly/ad hoc reporting, attending relevant training, and partnering with the designated points of contact. This position works with Congreso's Healthcare Services points of contact to obtain client referrals. This position reports to the Project Manager . This is a Hybrid position with onsite work conducted at Congreso's main office and/or a client's home. This position requires frequent, local travel, home visits, and program screenings, where appropriate. The Community Health Worker applies the principles of the organization's signature service delivery approach, Primary Client Model (PCM™), by promoting a positive employee/client-focused experience that is appreciative of the unique strengths, values and needs that each individual brings; and by using data to inform and drive decision making. Provides interventions to impact the social and environmental barriers to care of multi-generational households to include children, older adults, frequent utilizers of emergency health services, and those with frequent hospital admissions, and/or clients not meeting managed care standards for their chronic medical conditions (focus on Hypertension, Diabetes, HIV, and Asthma). Provides age-appropriate health education, emphasizing the importance of adult learning to promote healthy habits in younger generations. Works directly with members of the HCS division to identify clients needing additional support to close medical gaps and connect clients with necessary interventions and assistance. Prepares and participates in monthly case consultations with Congreso's Healthcare Services points of contact to review status of their client referrals to the program and establish next steps. Helps clients to connect and receive appointment reminders as necessary. Ensures touchpoints with identified clients are made and documented in Congreso's Efforts-to-Outcomes database. Submits client status reports to the Project Manager and internal points of contact as scheduled. Submits accurate and timely required funder reports to the Project Manager. Maintains required documentation of activities, participants, and program outcomes. Competencies (Knowledge, Skills, and Abilities) All Staff Competencies Basic understanding of Congreso's mission, values, DEI&B vision, Mission2Impact, Primary Client Model, programs, and services. Demonstrates knowledge and understanding of the community needs and demographics. Ability to understand, acknowledge, and assess client needs to provide a timely, trauma informed approach in service. Ensures the appropriate handling of sensitive information and understanding for issues such as confidentiality, domestic abuse, child abuse, and mandated reporter requirements. Adheres to all Congreso, departmental, programmatic, and security policies, procedures, and guidelines. Attends all Congreso trainings and in-services as required, including relevant professional groups to maintain professional, requisite knowledge and licensure. Attends, participates, and facilitates relevant staff meetings to promote communication and execution of goals. Ability to effectively use standard office equipment, including a variety of common software programs such as Office365, UNIDAD, and other customized databases. Follows through on assignments, works in a timely manner, within established deadlines, and completes special projects specific to the function of the department, as needed or as directed by supervisor. Demonstrates ability to work independently and collaboratively with diverse groups, in a compassionate, courteous, and respectful manner that fosters an inclusive environment. Strong written and verbal communication skills including effective and respectful communication with individuals and groups. Bilingual (Spanish/English) written and verbal skills a plus .

Minimum Experience, Education, and Qualifications Minimum Education:
High school diploma or
GED/HISET
required. College level course work a plus.
Minimum Experience:
Demonstrated experience working in a community-based and/or healthcare setting. Familiarity working as a member of an interdisciplinary team, a plus. Knowledge of some medical terminology, a plus
Certification/License:
Community Health Worker certification a plus. Valid driver's license, a plus. Child abuse and Criminal Clearances must be obtained prior to hire start date and updated every 2 years.
