Job Description
Job Overview Join our dedicated team as a Community Health Worker and become a vital link in promoting healthier, more empowered communities! In this role, you will engage directly with individuals and families to provide education, support, and resources that improve overall well-being. Duties Conduct outreach to community members to assess their health needs and provide tailored education on topics such as nutrition, addiction counseling, and preventive care Assist individuals with navigating healthcare systems, including Medicare enrollment and understanding medical terminology Support clients with disabilities by offering personalized assistance and connecting them with appropriate services Utilize motivational interviewing techniques to encourage positive behavior change and improve health outcomes Collect and document data related to community health initiatives, ensuring accuracy and confidentiality Deliver engaging public speaking sessions to educate groups on public health topics and available resources Collaborate with healthcare providers, social workers, and community organizations to coordinate care plans and follow-up services
Minimum Qualifications:
High school diploma or GED required. Valid driver's license and reliable transportation, required. Ability to communicate effectively. Strong organizational skills for health tracking, managing data, scheduling education, and working with community. Ability to work independently in community-based settings. CPR/First Aid certificationPreferred Qualifications:
College Education in Social Work, Nursing or Public Health Community Health Worker certification. Experience with rural populations or community-based outreach.
