You will provide personalized support, education, and resource navigation to individuals and families, empowering them to achieve better health outcomes. Responsibilities Conduct outreach and build rapport with community members to identify their health needs and concerns Educate individuals on topics such as nutrition, medication management, addiction counseling, and preventive health practices Assist clients with navigating healthcare systems, including Medicare and other social services programs Collect and document data related to client interactions, health status, and service utilization accurately and confidentially Apply motivational interviewing techniques to encourage behavioral change and enhance client engagement Collaborate with healthcare providers, social workers, and community organizations to coordinate care plans Deliver public speaking engagements or workshops on health topics to promote awareness and education Requirements Experience or background in nursing, social work, public health, or related fields Demonstrated ability to work effectively with individuals with disabilities or chronic conditions Strong communication skills for teaching, public speaking, and motivational interviewing Knowledge of medical terminology, addiction counseling principles, and nutrition fundamentals Familiarity with Medicare policies and healthcare systems navigation Ability to collect, organize, and analyze data accurately for reporting purposes Compassionate attitude with a passion for serving diverse populations in a community setting
