Justice/ReEntry Community Health Worker Coordinator Beacon Community Connections Lafayette, LA Job Details Full-time $52,000 a year 1 day ago Qualifications Employee relationship building Full Job Description Beacon Community Connections Justice Navigation Coordinator Job Description Updated 01/2024 The Justice Coordinator is the manager of the Community Care Hub for Reentry program and other assigned justice programs. They are responsible for the administration of Justice programs and services and provide direct supervision of any staff assigned to them. The position reports directly to the managing director. General Responsibilities Manage administration of service line or operational area and ensure all program, project, and contractual deliverables are met. Provide supervision and guidance to staff assigned to specific projects. Assist in data cleaning within SOCHR Engage in relationship building with community partner agencies and their personnel. Create, refine, and update project manuals, guides, and other tools for staff on at least an annual basis. Lead regular meetings (Huddles and 1:1) with staff Justice Navigation Responsibilities and Metrics Manage the entire social care process for Navigation. This includes lead generation, lead review, lead conversion, case openings, case interactions, case resource connections, and case closings. Provide supervision and guidance to Navigators through the social care process. Review and use SOCHR data to manage programs, personnel, and social care processes Track complex case resource needs and vet new resources in order to assist Navigators and improve client care and outcomes. Engage in relationship building with referral partners Engage in relationship building with resource partners agencies and their personnel. Create and update Justice Service Line written documents (e.g., Navigation Guide and Navigation Tools for Success) on an annual basis. Meet the goals, objectives, and deliverables for the CIG contract for Community Care Hub for Reentry programs, including, but not limited to: Submit annual goals and deliverables for establishing the baseline to determine sufficient compliance. Provide monthly progress reports detailing activities for that month, including updates to the Performance Measurement Plan to track the progress of achieving stated goals. The Year-end report shall be due by thirty (30) calendar days following the end of contract years 1 and 2. Meet the contract metrics of the CIG program. Beacon-wide Staff Responsibilities Complete all work with Beacon's mission and vision in mind, including assigned tasks or projects outside the scope of regular work responsibilities. Professional Qualifications Bachelor's degree required or management/supervisory experience (3-5 years minimum) Three or more years of management experience preferred Demonstrated ability to oversee and collaborate with staff Strong written and oral communication skills Ability to interface and engage diverse employees, volunteers, donors, and others Empathetic demeanor with clients and staff. Sensitive to the needs of the poor, uninsured, elderly, disabled, chronically ill, justice-involved, or other marginalized people Strong computer skills and the ability to accurately document client activities in an electronic health record Comfortable working in a busy, loud environment Monday-Thursday (in-office) 7:30 am - 4:15 pm Friday (Remote) 7:30 am - 12:30 pm
