About Malama Malama is a community-centered maternal health organization providing a comprehensive continuum of birth equity services to pregnant and postpartum Medi-Cal members across California. With a program reach of over 2,500 patients, Malama delivers doula services, Community Health Worker navigation, care coordination, prenatal and postpartum health education, and a CDC-approved Diabetes Prevention Program (DPP) designed for the postpartum population. Our model is built on culturally congruent, trauma-informed care — meeting members where they are: in their homes, clinics, and communities. Malama's work has produced measurable results, including a 38% reduction in pre-term labor rates and a 9% reduction in C-section rates compared to gestational diabetes mellitus (GDM) population averages. Position Overview The Malama Ambassador is a community-embedded birth equity advocate who serves as a trusted connector between high-risk pregnant and postpartum Medi-Cal members and the clinical, social, and peer support services they need. Embedded in WIC clinics, Federally Qualified Health Centers (FQHCs), and community-based organizations (CBOs) across the NorCal region, the Ambassador performs outreach, eligibility screening, and warm handoffs to Malama's Enhanced Care Management (ECM) Birth Equity services. Key Responsibilities Community Outreach & Engagement Conduct in-person outreach at WIC clinics, FQHC waiting rooms, and CBO program sites across assigned counties Build trusted relationships with pregnant and postpartum Medi-Cal members, particularly those who are Black, Indigenous, or from communities of color Distribute multilingual outreach materials (Spanish, Tagalog, Vietnamese, and other languages per county demographics) Represent Malama at community events including baby showers, health fairs, and partner-hosted convenings ECM Eligibility Screening & Referral Screen community members for Enhanced Care Management (ECM) Birth Equity eligibility per CalAIM criteria Conduct warm handoffs to Malama's care coordination, doula, and CHW navigation teams Coordinate with Kaiser Permanente care teams for KP Medi-Cal members to ensure complementary service delivery Support member enrollment and onboarding into Malama's ECM Birth Equity program Partner Site Integration Embed in partner site workflows at assigned WIC, FQHC, and CBO locations on a regular, scheduled basis Collaborate with partner site staff to co-develop and refine ECM referral pathways Participate in partner kickoff meetings, quarterly learning community sessions, and ongoing site check-ins Support MOU implementation and documentation of Ambassador activities at each partner site Documentation & Reporting Document all member interactions, referrals, and outcomes in Malama's PointClickCare electronic health record (EHR) system Track KP-specific metrics including NICU admission rates, ED utilization, and failed-to-keep (FTK) appointment data Contribute to baseline, progress, and final reporting requirements under the CalAIM IPP grant Participate in Malama team meetings, supervision, and ongoing professional development Qualifications Required Current Community Health Worker (CHW) certification OR equivalent community health experience Deep ties to and lived experience in the communities served (NorCal priority counties) Commitment to birth equity, reproductive justice, and culturally congruent care Strong interpersonal skills and ability to build trust with diverse populations Ability to travel regularly across assigned counties for partner site visits Comfort using electronic health record systems or willingness to learn Preferred Doula training or certification (Malama will cover doula training if not already completed) Bilingual or multilingual — Spanish, Tagalog, Vietnamese, or other languages spoken in NorCal priority counties Prior experience with Medi-Cal, ECM, WIC, FQHC, or community-based maternal health programs Familiarity with CalAIM or Medi-Cal managed care programs What Malama Offers $25.00/hour | Part-time (~15 hrs/week) — with opportunities to grow as the program scales Entry-level welcome — no prior CHW experience required if you have community roots and a CHW cert (or equivalent experience) Meaningful community impact — your work directly reduces maternal and infant health disparities Connection to a statewide network of birth equity advocates, doulas, and community health workers Supportive, mission-driven team culture How to Apply Ready to make a real difference in your community?
