Job Description

The Wellness Advocate Team Leader oversees the daily performance and development of the Wellness Advocate team, ensuring high-quality service delivery, productivity, and progress toward wellness and prevention goals. This role provides guidance and support for Community Health Worker (CHW) certification requirements, program interventions, and member outreach initiatives. The Team Leader helps strengthen community health efforts by supporting care and resource navigation, promoting preventive care and chronic condition management, and ensuring alignment with CHW best practices and core competencies. This position also collaborates across departments to improve operations, identify efficiencies, and enhance member outcomes and experiences.
Qualifications include:
High school diploma or GED equivalent Certification/training as a Community Health Worker preferred. Two (2) to three (3) years of work experience with patient navigation, care management, or any kind of direct service provision to patients in a community health/outreach, or social resource agencies. Two (2) years of experience as a lead, supervisor role, or equivalent experience. Computer literate in Microsoft Word, Excel, and Outlook. Strong analytical, organizational, prioritization and time management skills. Ability to work independently under minimal supervision.
