$19.66 - $34.59 Position Summary The Community Health Worker is an integral part of the oncology care team. Community Health Worker liaises between patients and clinic personnel to connect patients to community resources and optimize a patient's treatment journey. Community Health Workers are expected to assist with health literacy, cultural mediation, care access, and overall communication. Key Performance Areas KPA 1 — Patient Support & Care Coordination Serve as a non-clinical member of the oncology care team performing independent, interdependent, and dependent support activities. Prepare and educate patients prior to clinic visits regarding care plans and expectations. Follow up with patients on health management plans and established goals. Develop trusting and supportive relationships with patients and families throughout the cancer care journey. Advocate for patient needs and help coordinate appropriate services and support. KPA 2 — Resource Navigation & Community Linkages Connect patients to health-related services and community resources based on care plans and identified social determinants of health needs. Assist patients with accessing services, including appointment accompaniment and enrollment form support. Facilitate communication among patients, families, care teams, and community-based organizations. Promote appropriate utilization of available health and social support resources. KPA 3 — Communication, Documentation & Compliance Document patient interactions, activities, and outcomes in medical record and care coordination platforms. Attend required care team and program meetings. Maintain certification through programs approved by the Texas Department of Health and Human Services. Demonstrate professional accountability, confidentiality, and adherence to program and organizational standards. Position Qualifications Education High school Diploma or GED Certifications/Licenses Current Community Health Worker Certification Skills Bilingual Spanish or Vietnamese preferred
