Job Description

Community Health Worker Location:
Columbia Family Clinic;
Dayton, WA Schedule:
Full-Time | 36 - 40 hours per week
Compensation:
$20.00 - $26.50 per hour; based on experience and licensing Flexible Scheduling | Full-Time | Monday to Friday Join our team at Columbia County Health System! We are seeking a compassionate and driven Community Health Worker (CHW) who is passionate about supporting patients, addressing social determinants of health, and connecting individuals and families to essential resources in their homes. The ideal candidate builds strong relationships, communicates effectively, and works collaboratively to promote whole-person care.
What We Offer Status:
Full-Time, 36 - 40 hours per week
Schedule:
Monday - Friday Certified Nursing Assistant (CNA) Program eligibility
Benefits :
Medical/Dental/Vision & Basic Life Insurance; eligibility to participate in a 401K Plan; PTO & Sick Time accruals; six paid holidays Supportive team environment in a community hospital setting Qualifications Active Washington State Certified Nursing Assistant (CNA) license preferred but not required BLS certification every 2 years; will help obtain Completion of a 10-week Community Health Worker Program; will help obtain Requires travel to and from Clinics and patient homes Responsibilities Build supportive relationships with patients and families Assess social needs and connect patients to community resources Support access to food, housing, transportation, and healthcare services Provide follow-up through calls, visits, and care coordination Collaborate with care teams to address patient goals and barriers Advocate for patients and serve as a liaison with community agencies Document patient interactions and progress in the EMR Manage caseload, attend meetings, and maintain HIPAA compliance If you're a compassionate individual seeking a full-time opportunity, wanting to make a difference in the community, we'd love for you to join our team.
