This position offers the opportunity to make a meaningful impact on resident health and wellness through nutrition assessment, education, and care planning. The ideal candidate is compassionate, organized, and passionate about improving quality of life through individualized nutritional care. Responsibilities Conduct nutrition assessments and screenings for residents Develop, implement, and monitor individualized nutrition care plans Provide nutrition counseling and education to residents, families, and staff Monitor resident weights, nutritional status, and dietary interventions Collaborate with physicians, nursing staff, therapy teams, and dietary services Ensure compliance with state, federal, and facility regulations Participate in care plan meetings and interdisciplinary team discussions Review menus and dietary programs to support resident needs Maintain accurate and timely documentation Qualifications Registered Dietitian credential required Current licensure/certification as required by state regulations Experience in skilled nursing, long-term care, rehabilitation, or healthcare settings preferred Knowledge of clinical nutrition, therapeutic diets, and regulatory requirements Strong communication and organizational skills Ability to work independently while collaborating with interdisciplinary teams Benefits Competitive compensation Health, dental, and vision insurance Paid time off 401(k) with company match Professional development opportunities Supportive leadership team Join a team dedicated to providing exceptional care and improving the lives of those we serve.
