Conduct nutritional assessments and create individualized care plans Monitor dietary needs, preferences, and changes for residents Collaborate with our interdisciplinary team (nursing, culinary, social services & therapy) Ensure compliance with state, federal, and facility nutrition standards Educate staff, residents, and families on nutrition and wellness Support quality improvement initiatives for dietary services đź§ Who You Are:
A Registered Dietitian (RD/RDN) in good standing Experienced in long-term care or skilled nursing settings (preferred) A great communicator and proactive problem-solver Passionate about improving lives through nutrition Organized, friendly, and a true team player 💚 Why Rock Creek Care Center?
