Registered Dietitian Nutritionist Emmanuel Care Center-travis Fairfield, CA Job Details Full-time From $75 an hour 10 hours ago Benefits Health insurance Dental insurance Vision insurance Qualifications Bachelor's degree in nutrition and dietetics Dietetics Registered Dietitian Doctoral degree in nutrition Full Job Description Emmanuel Care Center-Travis are seeking a compassionate and detail-oriented Registered Dietitian (RD) to join our Skilled Nursing Facility team. The RD will be responsible for assessing residents' nutritional needs, developing individualized nutrition care plans, monitoring dietary interventions, and collaborating with the interdisciplinary team to support residents' overall health and quality of life. The ideal candidate is knowledgeable in clinical nutrition, long-term care regulations, and medical nutrition therapy, with strong communication and documentation skills. Responsibilities Complete comprehensive nutrition assessments for residents upon admission, quarterly, annually, and as needed. Develop, implement, and monitor individualized nutrition care plans. Evaluate residents for nutritional risk, weight changes, wounds, swallowing concerns, and special dietary needs. Collaborate with physicians, nursing staff, speech therapy, food service staff, and other interdisciplinary team members. Monitor meal intake, supplements, hydration status, and therapeutic diets. Provide nutrition education and counseling to residents, families, and staff. Ensure compliance with federal, state, and facility regulations related to dietary services. Participate in care plan meetings and quality assurance programs. Review and update nutrition documentation in the electronic medical record. Support food service operations and menu planning as needed. Qualifications Registered Dietitian credential (RD/RDN) required. Current state licensure/certification as required. Bachelor's degree in Dietetics, Nutrition, or related field. Experience in skilled nursing, long-term care, rehabilitation, or clinical nutrition preferred. Knowledge of CMS regulations and medical nutrition therapy. Strong assessment, communication, and organizational skills. Ability to work independently and as part of an interdisciplinary team.
