Registered Dietitian (RD) - Home Health Agency Guardian Angel Home Health Agency Inc Santa Clarita, CA Job Details Per diem $105 an hour 23 hours ago Qualifications Hospice experience Hospice care Driver's License Dietetics Registered Dietitian Full Job Description Guardian Angel Home Health Agency is seeking a compassionate and experienced Registered Dietitian (RD) to join our growing team serving patients throughout Los Angeles and Ventura Counties . As a member of our interdisciplinary team, you will provide nutritional assessments, education, and care planning to patients in their homes, helping improve health outcomes and quality of life. Responsibilities Perform comprehensive nutritional assessments for Home Health and Hospice patients. Develop individualized nutrition care plans based on patient needs and physician orders. Provide nutrition counseling and education to patients, families, and caregivers. Collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals. Monitor patient progress and revise care plans as needed. Complete timely and accurate documentation in accordance with agency policies and regulatory requirements. Participate in interdisciplinary team meetings and case conferences. Qualifications Current Registered Dietitian (RD/RDN) credential. Active California licensure (if applicable). Minimum one year of clinical experience preferred. Home Health or Hospice experience preferred. Strong assessment, communication, and documentation skills. Valid California Driver's License, reliable transportation, and current auto insurance. Ability to work independently and manage a flexible schedule.
