Job Description

Upland Rehabilitation Care Center is a state-of-the-art skilled nursing and subacute facility featuring 198 beds located in Upland, CA.
Duties:
Assist in planning regular and special diet menus as prescribed by the attending physician for individual residents. Review therapeutic and regular diet plans and menus to assure they are in compliance with the physician's orders. Encourage the resident/family to participate in the development and review of the resident's plan of care. Assist the Discharge Planning Coordinator in planning the dietary services portion of the resident's discharge plan. And more.
