Experience in SNF preferred! This is position is full-time and requires 40 hours per week. The ideal candidate should be comfortable working independently in a fast-paced environment, have experience doing full nutritional assessments, assessing for malnutrition, completing MDS/care plans, attending care conferences, enteral nutrition, and frequent monitoring of high-risk patients. The ideal candidate will have strong written and verbal communication skills and will work well as part of the interdisciplinary team. The RD is integral to the success of the entire Food & Nutrition Department. The ideal candidate will work closely with the dietary director and not hesitate to check in with the kitchen, conduct sanitation audits, etc. The RD will also participate in QA meetings for the facility, often representing the Food & Nutrition department. We offer a local support from sister buildings (we help cover your time-off!), and give you the freedom to make decisions that are best for your residents and facility. Experience in long-term care/skilled nursing required. Job duties include, but are not limited to: Assesses patient's nutritional status and needs (admission assessments, significant changes, quarterly and annual assessments) Develops and implements patient care plans and assesses patient responses Frequent monitoring of high risk patients: Enteral nutrition, TPN, pressure ulcers, dialysis, significant weight changes Participation in the MDS process, the interdisciplinary plan of care, and documentation of nutritional care plan in the medical record Provide nutrition education for patients and families Involvement in care conference meetings Collaborate with physicians, nursing, pharmacy and other healthcare team members/departments to determine, coordinate, and provide medical nutrition therapy Develop, modify or approve and make requested changes to facility menu Support the dietary director in compliance with all local, state and federal regulations Meal observations Functions as a preceptor in support of dietetic interns on occasion Experience in long-term care/skilled nursing required.
