Job Description

South Valley Post Acute is a premier skilled nursing and rehabilitation facility. We deliver exceptional transitional care, short-term rehab, and long-term care. Our team fosters a supportive environment focused on patient recovery and quality of life. We are seeking a dedicated, full-time Clinical Registered Dietitian to join our nutrition services team. You will oversee nutritional care for a diverse resident population. You will collaborate with a multidisciplinary team to optimize patient outcomes. Key Responsibilities Complete comprehensive nutritional assessments for new and existing residents. Develop customized medical nutrition therapy plans based on medical history. Participate in interdisciplinary care plan meetings and weight committee reviews. Manage residents requiring enteral nutrition, parenteral nutrition, and therapeutic diets. Monitor significant weight changes, skin breakdown, and lab values daily. Document clinical notes in the Electronic Health Record system accurately. Provide nutrition education to residents, family members, and facility staff. Oversee food service operations to ensure compliance with dietary regulations. Qualifications Bachelor's degree in Clinical Nutrition, Dietetics, or a related field. Current Registered Dietitian credential through the Commission on Dietetic Registration. One year of clinical experience in a skilled nursing facility preferred. Strong knowledge of state and federal regulations governing long-term care. Proficiency with Point Click Care preferred.
