Job Description

Job Summary We are seeking a passionate and dedicated Nutritionist to join our healthcare team, committed to promoting optimal health and well-being through personalized nutrition strategies. In this role, you will assess patients' nutritional needs, develop tailored care plans, and provide expert guidance to support recovery, weight management, and long-term health goals. Your expertise will help improve patient outcomes across diverse settings, including hospitals, long-term care facilities, and outpatient clinics. Responsibilities Conduct comprehensive patient assessments to evaluate nutritional status, including dietary habits, medical history, and physiological factors Develop individualized nutrition care plans that align with medical diagnoses and treatment goals Provide expert guidance on specialized nutrition therapies such as tube feeding and dietary modifications for acute care and geriatrics patients Collaborate with multidisciplinary teams to integrate nutrition strategies into overall patient care plans Monitor patient progress regularly, adjusting care plans based on diagnostic evaluations and evolving needs Educate patients and caregivers on nutrition principles, weight loss strategies, and lifestyle modifications for long-term health maintenance Document all assessments and interventions accurately within electronic medical record (EMR) systems Qualifications Bachelor's or Master's degree in Nutrition, Dietetics, or a related field; registration or certification as a licensed dietitian is preferred Proven experience in hospital settings or long-term care environments with familiarity in acute care and geriatrics Strong knowledge of physiology, diagnostic evaluation methods, and clinical nutrition practices Hands-on experience with dietary department operations, care planning, and patient assessment techniques Proficiency in EMR systems for documentation and communication purposes Demonstrated skills in health coaching, supervising staff or interns, and developing comprehensive care plans Familiarity with tube feeding procedures and nutritional support for diverse patient populations including weight loss programs Join us to empower patients through expert nutritional guidance!
