Assess, diagnose, and implement nutrition care plans tailored to each patient's needs. Monitor outcomes such as weight, labs, wound healing, and clinical progress.Educate & Empower:
Offer nutrition education to patients, families, staff, and the community. Develop patient-centered materials and resources that support understanding and long-term health. Collaborate withCare Teams:
Participate in rounds, patient care conferences, and interdisciplinary meetings. Communicate with providers to ensure safe, cost-effective, and high-quality nutrition therapy.Lead & Mentor:
Guide dietetic technicians, interns, and students in delivering nutrition care. Contribute to hospital projects, quality improvement initiatives, and policy development.Drive Quality & Safety:
Implement clinical projects, maintain the enteral formulary, and help advance patient safety goals in nutrition services.Qualifications Education:
High school diploma or GED, or equivalent.Experience:
1+ year working with frail or elderly populations- Preferred 6+ months transporting individuals with special mobility needs
- Strongly Preferred License(s)/Certification(s): Basic Life Support (BLS)
- American Heart Association, required within 30 days Valid Iowa Chauffeur's License
- Required upon hire Proof of Auto Insurance
- Required upon hire Child & Adult Abuse Mandatory Reporter Training (IA & IL)
- Required within 30 days
