Days | 8:00 AM•4:30 PM Assignment Overview UnityPoint Clinics•PACE Senior Care is seeking an experienced Registered Dietitian for a 13-week outpatient assignment within the PACE Day Center program. As part of the interdisciplinary care team, the Dietitian will provide nutritional assessment, care planning, and dietary education for senior care patients in an outpatient setting. This role requires strong clinical nutrition experience, EPIC EMR proficiency, and the flexibility to float between local campuses if needed.

Key Responsibilities Clinical Nutrition Services:

Conduct nutritional assessments and develop individualized care plans Monitor patient nutritional status and modify interventions as appropriate Provide dietary counseling and patient education

Interdisciplinary Collaboration:

Participate in care coordination with physicians and healthcare teams Support patient wellness goals within the PACE program

Documentation & Compliance:

Maintain accurate documentation within

EPIC EMR

Ensure compliance with facility and regulatory nutrition standards

Patient Support:

Educate patients and caregivers regarding dietary recommendations Promote healthy nutrition practices for outpatient senior populations

Requirements & Qualifications Experience:

Minimum 2 years of Dietitian experience required Outpatient or senior care experience preferred First-time travelers accepted

Licensure & Certifications:

Registered Dietitian Certification required Iowa License required BLS (AHA) required

Schedule Details Day Shift:
