Job Summary We are seeking a dedicated and energetic Dietitian to join our compassionate team. In this vital role, you will assess patient nutritional needs, develop tailored care plans, and provide expert guidance to promote optimal health outcomes. Your expertise will directly enhance the quality of life for our patients across various care settings. Responsibilities Conduct comprehensive patient assessments, including dietary and physiological evaluations, to determine nutritional needs. Develop and implement individualized nutrition care plans that address specific health conditions such as geriatrics, acute care, long-term care, and weight management. Provide expert guidance on specialized nutrition therapies, including tube feeding and dietary modifications for complex medical conditions. Collaborate with healthcare teams to integrate nutrition strategies into overall patient care plans and monitor progress regularly. Educate patients and their families on nutrition principles, health coaching techniques, and lifestyle modifications to support long-term wellness. Utilize electronic medical record (EMR) systems to document assessments, care plans, and progress notes accurately. Supervise or mentor junior staff or interns as needed, sharing your expertise in dietetics and patient care. Qualifications Bachelor's degree or higher in Dietetics, Nutrition, or a related field; registration with a recognized dietetic association preferred. Proven experience in hospital settings or healthcare environments with a focus on geriatrics, acute care, or long-term care. Knowledge of physiology and diagnostic evaluation techniques relevant to nutritional assessment. Hands-on experience with dietary department operations, EMR systems, and hospital protocols. Strong skills in health coaching, patient assessment, and developing comprehensive care plans. Familiarity with tube feeding procedures, weight loss programs, and dietary management for diverse medical conditions.
