Caribou Medical Center is hiring a Part-time Dietitian! Join our team as a Dietitian and make a meaningful impact on patient health every day! In this dynamic role, you'll provide evidence-based nutrition assessments, personalized counseling, and medical nutrition therapy to patients across acute, swing-bed, and transitional care settings. You'll play a key role in improving patient outcomes by partnering with an interdisciplinary healthcare team, contributing to quality and population health initiatives, and empowering patients, families, and our community through education and support. Core Responsibilities 1. Clinical Nutrition Care Deliver comprehensive, evidence-based nutrition assessments and develop individualized care plans—even for patients with complex medical needs. Provide medical nutrition therapy (MNT) aligned with physician referrals, clinical best practices, and patient goals. Track patient progress and adjust interventions to ensure optimal outcomes. Actively participate in interdisciplinary rounds, discharge planning, and patient care conferences. Maintain thorough and timely documentation in the electronic medical record in line with policies and regulatory standards. 2. Nutrition Education & Community Outreach Inspire and educate patients and families to support healing, wellness, and long-term health. Create and present engaging educational programs for community outreach, health fairs, and preventative health initiatives. Collaborate with departments across the organization to promote wellness and chronic disease prevention. 3. Interdisciplinary Collaboration Partner closely with physicians, nurses, case managers, therapists, and community organizations to ensure seamless, patient-centered care. Support smooth transitions by assisting with discharge planning, including coordination with outpatient providers and home health services. 4. Quality Improvement & Population Health Be a driving force behind quality improvement initiatives that enhance nutrition care, patient satisfaction, and clinical outcomes. Contribute to population health strategies, including malnutrition prevention, chronic disease management, and reducing readmissions. Help shape and refine nutrition-related policies, standards, and best practices. Additional Responsibilities Stay current by completing required education, skills, and competencies. Actively participate in meetings, trainings, and collaborative discussions—your voice matters! Follow hospital policies while contributing ideas to improve processes and outcomes. Support departmental and organizational goals as a dependable and positive team member. Maintain a clean, safe, and welcoming work environment. Bachelor's or Master's degree in Nutrition, Dietetics, or related field Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration State licensure (if applicable) Strong communication, teamwork, and critical-thinking skills Preferred:
Clinical nutrition experience in an rural care hospital Certified Diabetes Care and Education Specialist (CDCES)
