Job Description
GENERAL SUMMARY
Responsible for interpreting nutritional information and conveying such information to clients in a manner in which such information is comprehended. Responsible for teaching community participants, peer professionals and various lay groups.ESSENTIAL FUNCTIONS AND DUTIES
Demonstrated customer service by creating a welcoming environment. Assessed and affirmed progress on previous goal and/or secondary education, if applicable. Comprehensive diet assessment completed per state policy. Use of open-ended & probing questions demonstrated. Breastfeeding assessment completed and documented per policy. Nutritional risk assessment completed using the VENA (Value Enhanced Nutrition Assessment) process. Report and summarize certification, follow-up and secondary educational progress and activities on a weekly basis. Avoided extensive time spent on irrelevant information. CHP uses critical thinking to evaluate and prioritize health assessment(s). Assists participants in attaining goals by providing suggestions on " how to" actions/ideas and completed case notes As per state policy, the nutrition education provided met the specific needs of(prenatal, breastfeeding, postpartum, infant and children) Food Instruments issued for supplemental WIC Formula and Medical Nutritional Prescriptions included a warranting diagnosis (ICD if applicable), amount of formula needed, length of time that is medically required, the authorized supplement , date, name and authorized health professional requesting prescription. Develop a care plan for clients which includes any participant's request based upon current DHS guidelines.MINIMUM EDUCATION
Bachelor's degree required in Nutrition or DieteticsMINIMUM WORK EXPERIENCE
Excellent written and verbal communication skills.KNOWLEDGE, SKILLS & ABILITIES
Must have strong organizational skills. Excellent oral and written communication skills required. Knowledge of basic computer operations required.REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS
