Job Description

As a Registered Dietitian, you will provide overall nutrition care to patients of this healthcare facility. Conduct and implement comprehensive nutritional assessment for patients at nutritional risk; interpret metabolic and clinical data and screen, identify and monitor patients at nutritional risk; provide a variety of nutritional therapy intervention methods to resolve nutritional crisis and to ensure health maintenance. Coordinate all phases of nutrition care including nutrition assessment, care planning, monitoring, education and counseling of hospitalized patients and outpatients in a variety of settings. Perform duties in support of and in compliance with performance improvement plans, and other licensing, accrediting, and regulatory agencies.
ESSENTIAL FUNCTIONS
Develop and implement individualized dietary care plans utilizing nutritional assessment methodology to include planning individualized menus and calculating and planning modified diets to meet nutritional and psychological needs, ensuring conformance to licensed provider's orders and high standards of food quality. Interview patients and review medical and dietary history to determine food preferences and dietary habits and to assess nutritional needs; correlate information with patient's medical progress and plan meal patterns for the patient; make appropriate notations and recommendations on the patient's chart. Identify patients at nutritional risk based on established criteria. Evaluate age specific, developmental and disease specific nutritional needs of patients. Develop and implement the nutritional care plan. Evaluate and monitor the effectiveness and outcomes of medical nutrition therapy interventions. Evaluate effectiveness of dietary care plan in relation to patient's medical progress and modify or revise dietary care plan as required; prepare and maintain various records and reports pertaining to dietary services provided. Attend daily rounds and consult with physicians, nursing personnel and other members of the health care team to provide information regarding patient's dietary history, to ensure compatibility of the dietary care plan with other health care plans, to discuss unusual dietary needs or preferences of patients, to resolve problems and to implement changes. Function as an integral member of the clinical interdisciplinary team. Instruct patients and/or their families regarding hospital or home diets; develop and provide instructional materials to assist patients in their home dietary program; arrange for follow-up dietary care prior to discharge or transfer. Provide guidance to ancillary personnel engaged in writing diets as needed. Assist with implementing training manuals, policies & procedures as needed. Develop, prepare, and present educational materials used in the outpatient and inpatient setting for use by patients, families, and staff. Attend seminars and lectures to keep abreast of current research trends; participate as instructor in area of specialization in workshops, seminars, conferences and in-service education classes; provide instruction to interns and para-medical students. Perform other related duties incidental to the work described herein. Performs other related duties and assigned or requested.

EDUCATION AND LICENSURE
Work requires bachelor's degree in dietetics/nutrition and Academy of Nutrition and Dietetics registry. Licensed in the state of Kentucky is required. A minimum of one year in an acute care environment preferred.
