Job Summary Under the direction of the CDPA Diabetes Program Coordinator and within established organizational and departmental policies and procedures, responsible for providing individual consultation, assessment, and Diabetes Self Management Education and/or Medical Nutrition Therapy for referred diabetes program participants. QualificationsMINIMUM REQUIREMENTS
Bachelor's degree in food and nutrition or Dietetics required. Registration by the Commission on Dietetic Registration (CDR) required. Minimum 5 years' experience as an outpatient dietitian, particularly in diabetes management. Certification by the National Certification Board for Diabetes Educators. Additional Job Details (if applicable)ESSENTIAL JOB FUNCTIONS
Assist in the planning and review of the program each year. Evaluate the content and effectiveness of the program based on established clinical benchmarks. Coordination of community outreach activities such as program web site, physician meetings, and community education programs along with assistance/direction from Program Coordinator. Serve as a nutrition resource for providers and staff. Accurately assess physical, psychosocial, environmental, self-care, home needs and risk factors. Develop personal action plans for patients based on initial assessment, lab data and practitioner input. Establish individual goal plan with patients and evaluate progress/impediments. Adjust intensity of services based on patient's need related to goals and an appropriate utilization of resources. Provide individual nutrition consultation, assessment, and Medical Nutrition Therapy to referred diabetes program participants. Teach group classes according to established curriculum, covering basic diabetes education, stress management, risk factor reduction, diet, medication, blood glucose/urine testing, managing hypo//hyperglycemia, goal setting and other patient directed issues. Inform patient of necessary referrals to specialist, behavioral health, nutrition, podiatry, ophthalmology, and health education classes. Consult with primary care providers as needed and provide regular updates on their patients. Collaborate with other team members and Program Coordinator regarding patient care as necessary. Document outcomes in patient record. Complete documentation on all aspects of care. Update database registry. Maintain established hospital and departmental policies and procedures, objectives, confidentiality, quality improvement program, compliance, safety, infection control and environmental standards. Maintain professional growth and development through attendance at seminars, workshops, conferences or in-services, professional affiliations or journals to stay abreast of current trends in field of expertise. Meet annual competency and retraining requirements. Be a member of the Advisory Board and attend meetings as required. Regular and reliable job attendance is an essential job function. Perform other functions/duties as requested.
