Registered Dietitian Schedule :
Exempt (salaried), min 40 hours per week | Monday
  • Friday | 8am
  • 4pm or 9am
  • 5pm Occasional weekends, holidays, and department management coverage.

Dining Services Department What You'll Do:
The Registered Dietitian provides comprehensive nutrition care and education to residents within the skilled nursing community, including Porter Hills Village and Green House Homes. This role is responsible for assessing nutritional needs and developing, implementing, and monitoring individualized care plans to support resident health, safety, and quality of life.   • $72,000 / year Reporting To Brian Hughes Key Responsibilities Conduct comprehensive nutrition assessments for new admissions, readmissions, significant changes, and at required intervals.
Develop, implement, and monitor individualized nutrition care plans to support resident health outcomes. Identify and manage residents for nutritional risk. Monitor and document resident weights, food and fluid intake, laboratory values, and overall nutritional status; communicate changes to the interdisciplinary team. Recommend and oversee therapeutic diets, supplements, texture modifications, hydration interventions, and nutrition support, including enteral feeding. Collaborate with physicians and the interdisciplinary team, including nursing, therapy, and dining services, to support coordinated resident care and education. Participate in care plan meetings, committees, and quality improvement initiatives; contribute to performance improvement tracking and annual program review. Provide nutrition education to residents, families, caregivers, staff, and the community. Manage, review and approve menus to ensure nutritional adequacy, therapeutic compliance, and resident satisfaction; collaborate with culinary leadership on menu development and therapeutic diet implementation. Conduct regular meal rounds and dining observations to evaluate food quality, service, intake, and tray accuracy; address opportunities for improvement. Serve as a clinical and operational resource to dining and culinary staff regarding therapeutic diets and special nutrition needs. Participate in resident and family meetings as needed to support care and satisfaction. Support meal service and special events as needed, including assisting with meal setup, service, and approved substitutions. Skills, Knowledge & Expertise Active status as a Registered Dietitian Nutritionist (RDN) with the Commission on Dietetic Registration (CDR) and Academy of Nutrition and Dietetics (AND). Bachelor's Degree in Dietetics, Nutrition, Food and Nutrition or related field. Minimum of three (3) year working with the frail and elderly, preferred in a skilled nursing or senior living community. Knowledge of geriatric nutrition, medical nutrition therapy, and therapeutic diets. Experience with electronic medical records and nutrition documentation. Strong communication, organization, and time management skills and ability to work independently and collaboratively with interdisciplinary team.
