Under general direction, promotes individual and population health through advanced, comprehensive nutrition education, counseling and community-based initiatives. Provides advanced dietary and nutrition knowledge, counseling and recommendations for clients and families participating in Maternal Child Health programs in the clinic and/or home setting. Develops diet and nutrition plan of care based on a comprehensive assessment and client needs and interests. Serves as professional resource and nutrition expert participating in community-based programs, grants and coalitions. May staff WIC clinics serving as the WIC Nutrition Specialist, completing health and nutrition screenings and assessments for women, infants and children. Provides nutrition education, breastfeeding support and referrals for clients and families, Essential Duties Completes comprehensive health, growth and nutrition assessment of clients.

Provides client-centered counseling and referrals, engaging clients in setting goals and behavioral changes necessary to achieve optimal health status.

Promotes, protects and supports breastfeeding initiation and duration among clients and the community. Displays a positive attitude related to breastfeeding to clients and the general public.

Makes appropriate nutritional and breastfeeding recommendations to healthcare providers and maternal child health staff.

Works as team member in sharing information and knowledge, and coordinating client services and referrals to ensure effective health outcomes.

Documents accurate and detailed diet and nutrition care plan in electronic health record including billable visits. Ensures accuracy, integrity and confidentiality of record information and documents.

Provides breastfeeding support, education, intervention and referrals.

Evaluates, approves and documents special formulas for qualifying medical conditions.

Serves as nutrition expert and liaison for maternal and child health and other agency programs. Other Duties & Responsibilities Participates in multi-disciplinary teams including MIHP case conferences.

Participates in training and mentoring new staff and student interns.

Certified Lactation Educators or specialists: provide advanced breastfeeding interventions and recommendations.

Represents agency and public health at community coalitions.

Conducts outreach to community partners including health care providers.

Maintains up-to-date vaccines including flu (influenza,) in accordance with CDC guidelines: Recommended Vaccines for Healthcare Workers.
