Overview Our busy Endocrinology practice in Rochester Hills, Michigan is seeking a motivated

RN/NP/PA

to join our clinical team. This role is ideal for a professional with 2-3 years of experience in Diabetes care who enjoys building long term relationships with patients through Chronic Care Management. Duties Conduct thorough patient assessments to evaluate nutritional status, medical history, and lifestyle factors influencing health outcomes. Develop individualized nutrition care plans tailored to diabetic patients' specific needs. Provide expert guidance on diabetes management, weight loss strategies, and chronic disease prevention through evidence-based nutrition counseling. Educate patients on proper dietary practices, including carbohydrate counting, meal planning, and the use of dietary supplements when appropriate. Utilize electronic medical record (EMR) systems efficiently to document assessments, care plans, and progress notes accurately. Collaborate with healthcare providers to coordinate diagnostic evaluations, monitor treatment effectiveness, and adjust nutrition interventions accordingly. Stay current with emerging research in physiology, nutrition science, and healthcare regulations to continually enhance patient care quality. Experience Familiarity with EMR systems used for documentation and patient tracking within healthcare facilities. Strong understanding of dietary management for weight loss programs and dietary restrictions associated with chronic illnesses. Experience in developing comprehensive care plans

Pay:

$40.00 - $50.00 per hour
