Job Description

Join Our Team at Woodlake at New Hope! Woodlake at New Hope is seeking a compassionate, knowledgeable, and resident-focused Clinical Dietitian to join our healthcare team. If you are passionate about improving the health and well-being of older adults through nutrition, we would love to hear from you! Position Summary The Clinical Dietitian is responsible for assessing residents' nutritional needs, developing individualized nutrition care plans, and collaborating with the interdisciplinary team to promote optimal health outcomes. This role plays a key part in ensuring compliance with regulatory standards while enhancing the dining experience and quality of life for residents. Key Responsibilities Complete nutritional assessments and reassessments for residents. Develop, implement, and monitor individualized nutrition care plans. Provide nutrition counseling and education to residents, families, and staff. Monitor weight changes, lab values, dietary intake, and other nutritional indicators. Collaborate with physicians, nursing staff, speech therapy, and dietary services. Participate in care conferences and interdisciplinary team meetings. Ensure compliance with federal, state, and facility regulations. Review menus for nutritional adequacy and therapeutic diet requirements. Qualifications Bachelor's degree in Dietetics, Nutrition, or related field. Registered Dietitian (RD/RDN) credential required. Licensed Dietitian status as required by state regulations. Long-term care or senior living experience preferred. Strong communication, organizational, and interpersonal skills. Ability to work independently while collaborating effectively with a multidisciplinary team. Support quality improvement initiatives related to nutrition and hydration.
