As the Dietician, you will be responsible for comprehensive nutrition assessment, individualized care planning, and ongoing monitoring of residents' nutritional status. You will manage nutrition care for individuals with a variety of conditions, including chronic disease, rehabilitation needs, wound care, and dysphagia. A typical day may include conducting assessments on new admissions, reviewing weight and intake reports, attending interdisciplinary team meetings, and participating in care conferences to ensure nutrition considerations are integrated into overall care plans. You will provide medical nutrition therapy, develop and implement nutrition interventions, and collaborate with the care team on strategies to address malnutrition risk, weight changes, and special therapeutic diets. The role may involve coordination with speech therapy for texture-modified diets, advising on enteral nutrition when indicated, and working closely with nursing and dietary services to support safe, appropriate meal service. You will also contribute to quality improvement initiatives related to nutrition and regulatory compliance. The facility offers a structured environment with defined shifts, typically daytime hours, allowing for a predictable schedule while still providing flexibility to meet resident needs and support team-based care. Patient and resident ratios are designed to allow for thorough assessments, meaningful follow-up, and participation in ongoing care planning.
