The primary purpose of this position is to plan, implement, and evaluate community and systems-level programmatic initiatives at the department and county level; build and participate in collaborative cross-sector and community partnerships; serve as a public health advisor for county and community partners to support and promote healthy lifestyles, prevent and control disease, develop and design environmental and policy level interventions that support improved community nutrition, and provide complex clinical services to medically high-risk clients with one or more medical/nutritional problems as needed. This position involves program development and management, community partnership and engagement, and program and policy planning to meet the changing needs of the community. Essential Functions The following essential functions serve as a general guide for this position. Position functions are subject to change based on departmental needs and external factors including, but not limited to, regulatory requirements. Conducts comprehensive community outreach to expand access to nutrition education throughout Chatham County; Designs and delivers culturally appropriate nutrition workshops and outreach programs tailored for diverse populations; Provides accurate, effective, and evidence-based nutrition information to all clients and perform duties in compliance with all pertinent regulatory standards; Develops strategies and programming that increase nutrition knowledge, improving community health; Provides group education in community settings, including but not limited to wellness and healthy living programs; Coordinates the daily operations of medical nutrition therapy programming, including recruitment, monitoring of client status, data management, and medical record documentation; Meets with clients by appointment at the clinic location, or at a prearranged point of service, in order to provide individual and/or group billable services (diabetes education and/or medical nutrition therapy counseling); Reviews records of nutritional therapy clients for accuracy and completeness of necessary data, treatment, referral, and follow-up; Responsible for the application of the Nutrition Care Process, including completion of nutrition assessments, determination of nutrition diagnosis, development and implementation of nutrition interventions, and monitoring and evaluation of client progress and outcomes; Utilizes an electronic medical records system to monitor and follow clients who require chronic disease management; Attends internal Provider Team meetings and is an active member on the team; Develop strategies for the implementation and evaluation of departmental programs and action plans in collaboration with other public health department staff, county departments, and community-based organizations and coalitions; Identifies performance metrics to evaluate progress and outcomes for programs and strategies; When appropriate and needed, researchs and pursues grant and other funding opportunities to support the development and implementation of identified initiatives; Provides leadership in the utilization of educational concepts, methods, and materials and serve as a resource in clinical and community health services to the Public Health Department, other departments, the community, and stakeholders; Establishes and maintains community partnerships with key community groups to advance the priorities of the Chatham County Community Health Assessment health priorities; Pursues collaborative strategies that align cross-sector population goals and decrease duplicative efforts; Represent the Public Health Department on various committees (i.e., Chatham County Employee Wellness Committee) as it relates to the mission of the department and the health of Chatham County; Develops news articles and other marketing/educational materials, as needed, to increase community awareness of services available through the Health Department; Interprets concepts, purposes, and theories of public health, nutrition, and health promotion; Provides recommendations for program and policy development by researching evidence-based innovative practices; Maintains up-to-date knowledge, resources, and corresponding evidence-based interventions and practices, and serve as a preceptor for dietetic interns on occasion; Participates in continuing education pertinent to the position in addition to required departmental trainings; Participate in assigned activities as needed to maintain patient standards of care, as well as continuing education trainings to maintain dietetic licensure; Subject to be called upon at any time to help the agency respond to a public health emergency which may include, but is not limited to, outbreak investigations, staffing mass dispensing or immunization clinics, providing disaster relief, or other incidents (man-made or natural and intentional or non-intentional) as they arise, or any public health response requested by the Public Health Director; Contributes to accreditation documentation as requested; Performs other duties as assigned. Minimum Qualifications Registered Dietitian Nutritionist with the Commission on Dietetic Registration in North Carolina, with a Master's Degree in Public Health or Nutrition/Dietetics from an accredited college or university, and one year of relevant experience; or Registered Dietitian Nutritionist with the Commission on Dietetic Registration and two years of relevant experience. Prior experience in a public health setting is preferred. Fluency in Spanish. May consider a Registered Dietitian eligible candidate who can obtain their license within six months of employment. Knowledge, Skills, and Abilities Sufficient knowledge of program planning and evaluation; Skilled in building trusting relationships with patients across all cultural and socioeconomic backgrounds; Experience working with multiple agencies and organizations to develop effective collaborations and partnerships with community agencies and the public; Ability to write and speak effectively and to stimulate interest and cooperation among various groups in guiding a public health program; Ability to apply technical and systems knowledge of current nutrition principles, theories, and research to individual client care as well as to community programs and initiatives; Basic computer skills and the ability to use email, Microsoft Word, Excel, and electronic health record documentation; Physical Requirements This work requires the occasional exertion of up to 10 pounds of force; work regularly requires sitting, speaking, hearing and repetitive motions, frequently requires touching or feeling and occasionally requires standing, walking, climbing, balancing, reaching, pushing, pulling and lifting; work has standard vision requirements; work requires vocal communication to express or exchange ideas orally; work requires hearing to perceive information at normal spoken levels; work requires preparing and analyzing written or computer data; work has no exposure to environmental conditions; work is generally performed in a moderately noisy location (e.g. business office, light traffic). Special Requirements •Must be fingerprinted and successfully pass a Criminal History Record Check with the North Carolina State Bureau of Investigations (NCSBI)•Must possess a valid North Carolina driver's license. Must complete the following trainings within the first year of employment: Incident Command, Customer Service, Respiratory Protection and Fit, HIPAA, Policies and Procedures, Chatham County Public Health Department Orientation, Chatham County Orientation, and CPR. Must complete annual trainings, including Cultural Competency, Bloodborne Pathogens, Chatham County Public Health Department HIPAA, Public Health Laws, Policies and Procedures, Competency, Mandatory Reporting Laws, and Customer Service Training. Must participate in appropriate and approved continuing education to maintain license and CPR certification. Must maintain active registration as a Registered Dietitian Nutritionist (RDN) through the Commission on Dietetic Registration and active licensure with the North Carolina Board of Dietetics/Nutrition. Ability to speak Spanish fluenty.
