Job Description

Job Summary Trinity Health is seeking a dedicated Registered Dietitian to provide nutritional care services within a Long-Term Care setting. The Dietitian will assess residents' nutritional needs, develop individualized nutrition care plans, monitor outcomes, and collaborate with interdisciplinary teams to support optimal resident health and wellness. This position plays a vital role in improving patient outcomes through evidence-based nutrition interventions and education. Responsibilities Conduct comprehensive nutritional assessments using clinical data, laboratory values, weight trends, and disease-specific guidelines. Develop, implement, and modify individualized nutrition care plans. Recommend therapeutic diets, nutritional supplements, enteral feeding, and hydration strategies. Monitor resident progress and adjust interventions based on changing medical conditions. Collaborate with physicians, nursing staff, and interdisciplinary care teams. Provide nutrition education and counseling to residents, families, and staff. Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements. Participate in quality improvement initiatives, audits, and survey readiness activities. Support community outreach and educational programs as needed. Ensure compliance with professional, state, and federal standards of practice. Requirements Registered Dietitian (RD) credential through the Commission on Dietetic Registration (CDR) required or eligible for registration. North Dakota Registered Dietitian License required or eligible upon hire. Candidates eligible for registration must obtain RD status within 6 months of hire. Minimum 2 years of Dietitian experience preferred. Long-Term Care experience preferred. Strong clinical nutrition assessment and care planning skills. Knowledge of therapeutic diets and disease-specific nutrition interventions. Excellent communication and patient education abilities. Ability to work effectively within an interdisciplinary healthcare team.
Pay:
$2,320.00 - $2,378.00 per week
