Job Description

Director of Clinical Nutrition -Full Time-Day The Director of Clinical Nutrition provides quality nutrition care and education to patients and family members. Director of Clinical Nutrition works closely with the interdisciplinary team to assess the nutritional status of patients and recommends the appropriate nutrition therapy as it pertains to the patients' nutritional needs. Provides nutrition counseling to patients and family members as indicated. The Director of Clinical Nutrition is responsible for activities that support patient-focused care within their assigned patient units. The Director of Clinical Nutrition also participates in coordinating departmental initiatives and education programs. Adheres to Silver Lake philosophies, standards, objectives, and policies and procedures. Responsibilities Evaluates the nutritional status of patients by reviewing pertinent information obtained from the electronic medical record and the interdisciplinary healthcare team. Applies knowledge of nutrition screening and assessment to specific patient populations to determine their nutritional needs. Conducts nutrition-focused physical exams. Develops nutrition plans for all residents requiring tube-feeding; i.e. those with ventilator care, trache care, dialysis care and wound care. Develops population-specific skill set in clinical nutrition assessment. Documents pertinent findings in the medical record as per policy. Provides quality nutrition counseling to patients and family members as indicated. Evaluates nutrition care plans for effectiveness and recommends changes as needed. Uses professional judgment to adapt and apply guidelines, protocols and professional standards of care to the new and changing needs of the patient. Participates in interdisciplinary rounds as needed. Communicates effectively with members of the healthcare team. Assists with the training and development of clinical nutrition staff. Assist with discharge planning as needed. Precepts dietetic interns and instructs them on the competencies required to complete their assigned rotation. Completes student evaluations. Coordinates projects within the department, including providing education via nutrition-related in-services, interdepartmental nutrition education programs and lectures. Assists with the development of and updates to nutrition education materials. Presents department journal clubs. Other duties as assigned.
Qualifications Education:
Bachelors degree or higher from an accredited institution, with completion of ACEND (Accreditation Council for Education in Nutrition & Dietetics)-accredited dietetic internship.
Experience:
2 years minimum of experience, including dietetic internship.
Licensing/Certification:
Must be a registered dietitian (RD) from the Commission on Dietetic Registration (CDR). We always take the time to get to know our residents and develop an individualized care plan that will fit their specific needs. We never take a one size fits all approach. The Finest Respiratory and Ventilator Therapy In 1991 Silver Lake Specialized Care Center was commissioned by the NYS Dept. of Health to develop a long-term respiratory ventilator program, in a post hospital setting. Our dedicated staff has been perfecting the concept ever since. Our unique approach is one that has been proven over the years, providing an overall wellness program to support independent breathing. We created the concept and we have been perfecting it ever since. Our program may be slow and steady, but it provides one with an excellent quality of life during the rehab process and a much more stable outcome when one is weaned.
