Registered Dietitian (PACE Program) East Brunswick, NJ Full-Time | Onsite $73,000 - $78,000 and Comprehensive Benefits About the Opportunity We are partnering with a respected PACE (Program of All Inclusive Care for the Elderly) organization in East Brunswick, NJ that is seeking a compassionate and clinically strong Registered Dietitian to join its interdisciplinary care team. This is an excellent opportunity for a Dietitian who is passionate about serving the elderly population and making a direct impact on participant health, independence, and quality of life. Position Summary The Registered Dietitian is responsible for assessing the nutritional status of PACE participants and developing individualized nutrition interventions based on clinical data, medical conditions, and participant preferences. This role serves as an active member of the Interdisciplinary Team (IDT) and plays a key role in care planning, participant education, and ongoing reassessment to ensure optimal health outcomes. Key Responsibilities Provide high quality nutritional care and services to PACE participants as a member of the IDT Conduct comprehensive nutritional assessments upon enrollment and as needed per PACE regulations Collaborate with the IDT to develop, implement, and revise individualized care plans Provide nutrition counseling and education to participants, families, and caregivers Monitor weight, hydration, dietary intake, mood changes, chewing/swallowing issues, drug nutrient interactions, and skin integrity Recommend and coordinate nutrition related interventions such as home delivered meals and supplements Communicate participant progress and nutrition related changes to the IDT in a timely manner Participate in IDT meetings, family meetings, in services, training, and quality improvement initiatives Incorporate cultural and ethnic food preferences into care planning Assist IDT members and center staff in understanding participants' nutritional needs Perform Food Service Supervisor duties as assigned Maintain accurate, thorough documentation and comply with HIPAA, OSHA, and PACE infection control policies Perform other related duties as assigned Qualifications Bachelor's degree in Food, Nutrition, or Dietetics from an accredited institution Registered Dietitian (RD) credential through the Commission on Dietetic Registration (CDR)
NJ:
Must be registered or eligible for registration with the CDR (Commission on Dietetic Registration) Minimum 1 year of experience working with the elderly population (training provided if not present) 2+ years of clinical nutrition experience preferred ServSafe certification preferred Why This Role?
